I have read and understand the Wyoming Fine Arts Center Guidelines outlined in this document. I understand that there is an inherent risk of exposure to COVID-19 that exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to severe illness and death. According to the Centers for Disease Control and Prevention, senior citizens and guests with underlying medical conditions are especially vulnerable. By visiting and sending my camper to the Wyoming Fine Arts Center I voluntarily assume all risks related to exposure to COVID-19. The Center will not be held responsible if the camper contracts COVID-19 while at the facility.

I am voluntarily participating in camp(s) at The Center and am aware that I am participating at my own risk. I assume all related risks, both known and unknown to me, of my participation in camp(s). I also recognize that The Center is not liable for any damage to clothing, and other personal belongings and assume all risks of any damage and loss that might occur.

In the event that I should require medical care or treatment, I agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance.

In the event that any damage to equipment or facilities occurs as a result of my or my family's willful actions, neglect or recklessness, I acknowledge and agree to be held liable for any and all costs associated with any actions of neglect or recklessness.

I understand that I will only receive a full refund if the camp I registered for is cancelled by The Center due to low enrollment. I understand that deposits and payments made can be used as a credit toward another camp, class, or lessons, and will not be refunded.
If a child is sick, they will not be allowed at camp, and the payment can be credited to another week, class, or lessons.
